Granted, it's a long fast, 48 hours.....but, Figure 5 in the Free Full-Text gets at the issue.

These researchers are delving into the complexity of tissue level expression of IGF-1, a truly mysterious part of the story that is still being worked out. Some tissues maintain local IGF-1 production irrespective of acute variations in serum IGF-1, while other tissues seem to "take their cue". And then some tissues, like muscle, stick IGFBP-5 out there.....

Very complicated....for the moment, I would watch serum IGF-1 more than anything.
