I thought this recent study in the March 2006 issue of Metabolism was particularly insightful into reasons why exercise may not work "as well" as interventions like caloric restriction.

In this study, the investigators tried to "titrate" the amount of exercise to the amount of food the person was eating, and have them "follow each other" as they walked the amount of food up to an "overfeeding" amount. In other words, if you eat more, can you just exercise it away?

Well, not entirely. They showed that fasting insulin levels did not lower to "baseline" when you exercise an amount that would be expected to "remove" the extra energy. In fact, they were a little bit higher than the group that just ate more.....

But, the "area under the curve" for insulin after a meal was reduced considerably by the exercise.

All in all, the fasting insulin result may indicate that they did not titrate things as well as they thought, but, I think in general it indicates that attempting to titrate exercise to the standard American diet is only a "half solution" to the problem. The reason this is so may have to do more with the fact that it is too hard to actually calculate and implement what would be most beneficial, but, there is a distinct possibility that exercise is simply incapable of producing the necessary endocrinological milieu for maximal lifespan extension.
